Option Care Health is seeking an Infusion Nurse II to deliver intermediate nursing care in the home or infusion suite, coordinating with physicians and the clinical team to ensure safe, compliant treatment under a plan of care. This position covers infusion suite and home visits with opportunities for growth.

Responsibilities include patient assessment, medication administration, patient and family education, and timely documentation to support billing and continuity of care.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year nursing experience.
• Current CPR certification required.

Responsabilités

• Initiates, develops, and implements intermediate nursing plan of care; administers medications and educates patients and families.
• Assesses patient needs and status during visits; re-evaluates and updates physician orders.
• Provides training and mentorship to Infusion Nurse I’s to ensure safety.
• Communicates with clinical team and documents care in real time for billing and continuity.
• Coordinates discharge planning and prepares summaries for transitions of care.
• Adheres to agency policies, HIPAA regulations, and professional standards.
