Job summary

Option Care Health is seeking an Infusion Nurse II to provide intermediate direct patient care in home or infusion suites under a physician plan of care. This role requires an active RN license, CPR certification, and strong documentation and teamwork skills.

The nurse will coordinate treatments, assess patient status during visits, mentor lower-tier nurses, and ensure compliant, high-quality care while maintaining HIPAA and company policies. Travel to patient homes is required.

Qualifications

• Active RN license in the state of practice.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit and documents changes for physician review.
• Provides training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
• Maintains timely clinical documentation and coordinates discharge planning and patient education.
