Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in patients’ homes or infusion suites. You will work under supervision to ensure safety and continuity under a physician's plan of care.

The role includes administering medications, educating patients and families, and ensuring timely clinical documentation for billing. A nursing license, relevant experience, and CPR certification are required.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit and communicates changes to the physician.
• Provides training and mentorship to Infusion Nurse I's to ensure patient safety and compliance.
• Delivers patient education using a patient-centered approach to promote independence with prescribed therapy and care needs.
• Documents care in real-time, coordinates discharge planning, and reports progress to the nursing team.
