Job summary

Soleo Health is seeking a Full-time Infusion Nurse to support our infusion patients in the Grayson, GA area. The nurse will provide primary patient care, including assessments, care planning, and education for infusion therapy in home or alternate sites, following care matrices and best practices.

Responsibilities include coordinating with pharmacists and care partners, maintaining documentation, and participating in on-call rotations while commuting to patient homes as needed.

Qualifications

• Current RN license in good standing.
• BSN preferred.
• Advanced certification (CRNI, ONS, IgRN) preferred but not required.
• Experience with on-call rotations including nights and weekends.
• Familiarity with infusion therapy and home care settings.

Responsibilities

• Provide direct care for all assigned patients including initiation and maintenance of communications with appropriate care team members.
• Evaluate and educate patient/care partner to safely administer home infusion therapy.
• Evaluate the home environment and identify interventions for safety and appropriateness of prescribed care.
• Develop care plans with the team and evaluate response to therapy.
• Manage schedule via company platform and communicate changes promptly.
• Coordinate patient care via approved company platform.
• Participate in nurse on-call rotation to ensure continuous nursing support.
• Document all patient encounters and care activities per Soleo policy and regulatory requirements.
• Maintain knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of POTs and required clinical documents per policy and state guidelines.
