Job summary

Soleo Health is seeking a Full-time Infusion Patient Care Coordinator (on-site) in Centennial, CO to support the specialty infusion pharmacy. The role involves data entry for billing, scheduling nursing activities, and coordinating patient care across teams to ensure a seamless experience.

Responsibilities include handling referrals, verifying benefits, and communicating with patients and providers. The position emphasizes exceptional customer service, scheduling accuracy, and coordination

Qualifications

• High school diploma or equivalent is required.
• Licensed Practical Nurse/Licensed Vocational Nurse license is preferred.
• Prior healthcare scheduling experience is required.
• Home infusion experience is required.
• Ability to work on-site and interact directly with patients in a clinical setting.
• Strong communication skills are required.
• Ability to prioritize and multitask.
• Basic computer skills including Excel, Word, Outlook.
• CPR Plus application experience preferred.

Responsibilities

• Receive medication referrals and collect insurance information.
• Proficient data entry and generating daily office communications.
• Contact referral sources, patients, or prescribers to obtain documentation for benefits or prior approvals.
• Place outbound calls to patients or doctor’s offices to notify care status.
• Act as a single point of contact for referral activity with internal customers, manufacturers/HUBs, patients, prescribers, and referral sources.
• Coordinate scheduling activities for the nursing department, ensuring accuracy and efficiency.
• Serve as the main point of contact for the nursing department, multi-tasking and remaining calm under pressure.
• Ensure patient understanding of their next visit and scheduling for timely dispensing of medications.
