Soleo Health is seeking an Infusion Patient Care Coordinator to support our Specialty Infusion Suite in Houston, TX. This role involves data entry into the clinical management system, coordinating nursing schedules, handling referrals, and ensuring a smooth patient experience.

Qualifications

• High school diploma or equivalent required.
• Licensed Practical Nurse/Licensed Vocational Nurse license preferred.
• Prior healthcare scheduling experience required.
• Home infusion experience required.
• On-site work requirement; no remote work.

Responsibilities

• Enter clinical information into the clinical management system for billing.
• Coordinate scheduling activities for the nursing department.
• Respond to incoming calls and manage referrals.
• Act as a single point of contact for referral activity with internal/external partners.
• Provide excellent customer service and ensure timely care coordination.
