Soleo Health is seeking a Full‑time Infusion Patient Care Coordinator in Centennial, CO to support our Specialty Infusion Pharmacy. You will handle data entry for billing, coordinate scheduling, and assist nursing with referrals and patient follow-ups.

The role emphasizes care coordination, responsive communication, and accurate patient scheduling, with a focus on timely medication dispensing and exceptional service.

Qualifications

  • High school diploma or equivalent required.
  • Licensed Practical Nurse/Licensed Vocational Nurse License preferred.
  • Prior healthcare scheduling experience required.
  • Home infusion experience required.
  • Ability to work on-site and interact directly with patients in a clinical setting.
  • Strong communication skills.
  • Ability to prioritize and multitask.
  • Basic computer skills including Microsoft Excel, Word, Outlook.
  • CPR Plus application experience preferred.

Responsibilities

  • Receive medication referrals and collect insurance information.
  • Perform proficient data entry and generate daily office communications.
  • Contact referral sources, patients, or prescribers’ offices to obtain additional information required for verification of benefits or prior approvals.
  • Place outbound calls to patients or doctors’ offices to notify them of care status.
  • Collaborate with prescribers to facilitate payor denial appeals.
  • Act as a single point of contact for all referral activity with internal customers, manufacturers/HUBs, patients, prescribers, and referral sources.
  • Provide exceptional customer service to external and internal customers, resolving requests in a timely and accurate manner.
  • Ensure appropriate notification of patients regarding financial responsibility, benefit coverage, and payor authorization for services.
  • Serve as the main point of contact for the nursing department, demonstrating the ability to multi‑task and remain calm under pressure.
  • Coordinate and plan scheduling activities for the nursing department, ensuring accuracy and efficiency.

Position Overview

The Infusion Patient Care Coordinator is responsible for data entry of clinical information into the clinical management system to support billing, coordinating scheduling activities, and handling associated tasks for the nursing department. The role includes care coordination, responding to calls, promoting agency programs, and processing referrals. The coordinator ensures accurate scheduling for all patient care teams and engages directly with patients following AIS visits to coordinate next steps in care and schedule appointments.

  • Demonstrate knowledge of the geographical area for efficient scheduling and coordination of nursing visits.
  • Clearly communicate patient needs for external agency support, when necessary, to ensure timely care.
  • Collaborate with internal departments and external partners to promote, foster, and maintain optimal relationships across departmental and company lines.
  • Support client satisfaction at a level that ensures account retention.
  • Meet with patients in person following AIS visits to schedule their next appointment prior to departure.
  • Confirm patient understanding of their next visit and ensure accurate scheduling to support timely medication dispensing and care coordination.
  • Inquire about the patient’s appointment experience and request completion of a brief satisfaction survey.
  • Serve as a key touchpoint for enhancing the in‑clinic patient experience and supporting overall customer satisfaction outcomes.
Schedule
  • Full time, 40 hours per week, Monday to Friday, 9:00 am – 5:30 pm.
  • This is a 100% onsite position; remote and hybrid work are not available.
  • Healthcare scheduling experience preferred.
