Job summary

Alivia Health, LLC is seeking an Infusion Admitting / Evaluating Nurse for a full-time role in Ponce, PR. Qualifications

• Bachelor’s degree in nursing from an accredited university.
• Minimum of 2 years of experience in home health, infusion, or acute care services, preferably in a home setting.
• Knowledge of infusion pumps, central and peripheral catheters.
• Ability to obtain informed consent and educate patients and families.

Responsibilities

• Conduct initial home assessments to determine feasibility and safety of infusion treatment.
• Admit patients within 24 hours of referral in accordance with standards.
• Coordinate medication reconciliation and develop the initial care plan with the prescribing physician and team.
• Monitor patient response and document findings in the electronic system.
