Job summary

Option Care Health seeks an Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or alternate infusion suite, ensuring patient safety under a physician's plan. Works under supervision and adheres to policy.

The role requires a valid RN license, 3–5 years infusion nursing experience, CPR certification, and the ability to travel as needed. Training and mentorship are provided to junior nurses and care teams.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 3-5 years of infusion nursing experience in home or ambulatory infusion settings.
• Current CPR certification required (in-person class per policy).

Responsibilities

• Initiates, develops, and implements intermediate nursing plans of care and educates patients and caregivers.
• Assesses patient needs at each visit and communicates significant changes to physicians.
• Provides training and mentorship to Infusion Nurse I and other staff as needed.
• Documents care in real-time and ensures billing timelines.
• Coordinates discharge planning and provides complete discharge summaries.
