Job summary

AbsoluteCare is seeking an experienced Advanced Practice Provider to deliver comprehensive outpatient HIV and chronic infectious disease care within a large integrated team. You will develop comprehensive care plans, educate patients, and coordinate services across the continuum of care to improve outcomes.

You will collaborate with social workers, case management, behavioral health, pharmacy, nutrition, and other providers in a PCMH-aligned model, ensuring high-quality, patient-centered care.

Qualifications

• Unrestricted Advanced Practice License in the state of Practice.
• Credentialed by AAHIVM.
• Minimum 2 years’ experience as Advanced Practice Provider.
• BLS certification.
• DEA registration.
• Experience in population health management preferred.
• Experience with psychosocially and medically complex patients preferred.
• Knowledge of NCQA PCMH model preferred.
• Ability to work in a multidisciplinary team.
• EMR systems experience and reporting capabilities.
• Proficiency with Microsoft Office Suite (Word, Outlook, PowerPoint, Excel).

Responsibilities

• Provide outpatient care for people living with HIV and chronic infectious diseases.
• Develop diagnostic and treatment care plans with the team.
• Coordinate transitions of care across hospital discharges, ER, SNFs, and home care.
• Collaborate with social work, care management, behavioral health, pharmacy, nutrition, and other team members.
• Educate patients about chronic diseases and advanced directives.
• Practice evidence-based medicine per the PCMH model of care.
• Lead a multidisciplinary team and implement plans of care from team members.
• T effectively manage and track patient panels to reduce ER visits and readmissions.
