Job summary

AbsoluteCare seeks a physician to deliver HIV care and management of chronic infectious diseases in an outpatient setting. You will develop diagnostic and treatment plans, counsel patients, and coordinate with a broad care team to ensure whole-person care.

Responsibilities include outpatient consultations, ongoing disease management, and collaborative care planning with PCPs, specialists, and care managers. A focus on transitions of care and patient education is essential.

Qualifications

• Unrestricted Medical License in the state where practice occurs.
• Board certification in Internal Medicine or Family Practice.
• AAHIVM certification or Infectious Diseases board certification.
• BLS certification.
• DEA registration.

Responsibilities

• Provide outpatient care for people living with HIV and related chronic infectious diseases.
• Develop and implement diagnostic and treatment plans.
• Collaborate with a multidisciplinary team to manage patient care and transitions.
• Educate patients on disease progression, treatment options, and advance directives.
• Coordinate care across hospital, ER, and home care settings.
• Utilize evidenced-based medicine within the PCMH care model.
