Job summary

AbsoluteCare is seeking a board-certified physician to provide HIV and chronic infectious disease care in an outpatient setting. You will develop diagnostic and treatment plans, educate patients, coordinate with a multidisciplinary team, and guide patients through care options and referrals.

Qualifications

• Unrestricted Medical License in the state of practice.
• Board certified Internal Medicine or Family Practice.
• AAHIVM certification or Infectious Diseases board certification.
• BLS and DEA certifications.
• Experience in population health management is preferred.
• Experience with psychosocially and medically complex patients is preferred.
• Knowledge of NCQA PCMH model is preferred.
• Ability to work in a multidisciplinary team.
• Strong communication and interpersonal skills.
• Proficiency with EMR systems.
• Microsoft Office proficiency.

Responsibilities

• Provide outpatient HIV and chronic infectious disease care.
• Develop and manage patient treatment plans.
• Collaborate with social work, care management, and other providers.
• Educate patients on disease management and care options.
• Lead and participate in multidisciplinary team care.
• Implement care plans and transitional care.
• Monitor patient outcomes to reduce ER visits and readmissions.
• Utilize evidence-based medicine within PCMH model.
• Triage urgent appointments and manage acute issues.
• Maintain accurate documentation and reporting.
