Job summary

AbsoluteCare Medical Center & Pharmacy in Chicago provides outpatient HIV and chronic infectious disease care within an integrated team. The Advanced Practice Provider will develop diagnostic and treatment plans, educate patients, and coordinate care across the continuum.

Qualifications

• Unrestricted Advanced Practice License in the state of Practice.
• Credentialed by AAHIVM.
• Minimum of 2 years’ experience practicing as an Advance Practice Provider.
• BLS.
• DEA.
• Experience with NCQA PCMH model preferred.
• Ability to work in a multidisciplinary team with collaboration across provider teams.
• Proven ability to leverage EMR reporting capabilities.

Responsibilities

• Provide care for people living with HIV and chronic infectious diseases.
• Outpatient care consultation and ongoing treatment of chronic infectious diseases.
• Deliver comprehensive care for patient panels, coordinating with primary care physicians and specialists.
• Work in a team environment with social work, care management, behavioral health, pharmacy, nutrition, etc.
