Job summary

Qualifications

• Unrestricted Advanced Practice License in the state of practice.
• Credentialed by AAHIVM.
• Minimum of 2 years’ experience as an Advanced Practice Provider.
• BLS certification.
• DEA registration.
• Population health management experience preferred.
• Experience with psychosocially and medically complex patients preferred.
• Knowledge of NCQA PCMH model preferred.
• Ability to work in a multidisciplinary team.
• Innovative self-starter with problem-solving skills.
• Experience maximizing EMR reporting capabilities.
• Proficient with Microsoft Office tools.

Responsibilities

• Provide outpatient care for people living with HIV and chronic infectious diseases.
• Develop diagnostic and treatment plans in collaboration with the care team.
• Educate patients about chronic diseases and healthy choices.
• Coordinate care across hospital discharges, ER, skilled nursing, and home care.
• Lead a multidisciplinary team including social work, care management, and pharmacy.
• Implement plans of care in collaboration with patients, families, and team members.
• Maintain patient panels and track disease progress to reduce ER visits and hospitalizations.
