Job summary

Oak Glen Post Acute in Cherry Valley, CA is seeking an Infection Preventionist to lead our infection prevention program, ensuring compliance with federal, state, and local regulations and promoting resident safety.

You will conduct surveillance, educate staff on hand hygiene and PPE, coordinate with clinical teams, and prepare reports for QAPI. California RN or LVN license preferred; CIC certification encouraged.

Qualifications

• California RN or LVN license required or preferred.
• Experience in infection prevention in healthcare settings.
• Knowledge of CDC, CMS, CDPH regulations.
• CIC certification preferred or willingness to obtain.

Responsibilities

• Develop, implement, and maintain the facility's Infection Prevention and Control Program.
• Conduct surveillance, tracking, trending, and reporting of infections and communicable diseases.
• Monitor and investigate infection outbreaks and implement corrective actions.
• Ensure compliance with CDC, CMS, OSHA, and state infection control regulations.
• Collaborate with nursing leadership and interdisciplinary teams to reduce healthcare-associated infections.
• Educate and train staff on infection prevention practices, hand hygiene, PPE usage, and isolation precautions.
• Maintain infection control documentation and prepare reports for Quality Assurance and Performance Improvement (QAPI) meetings.
• Conduct audits and provide ongoing monitoring of infection prevention practices throughout the facility.
• Assist with emergency preparedness and outbreak response planning.
• Serve as a resource for residents, families, and staff regarding infection prevention initiatives.
