Job summary

Oak Glen Post Acute in Cherry Valley, CA is seeking a dedicated Infection Preventionist (IP) to lead and coordinate our infection prevention and control program. This role will ensure compliance with CDC, CMS, OSHA, and state regulations, while promoting a safe environment for residents, visitors and staff.

The IP will conduct surveillance, educate staff, and support QAPI initiatives, with an emphasis on preventing healthcare-associated infections and improving outcomes.

Qualifications

• Current California RN or LVN license preferred.
• Infection Preventionist experience in a skilled nursing facility or healthcare setting preferred.
• Strong knowledge of CDC, CMS, and California Department of Public Health regulations.
• CIC certification preferred or willingness to obtain.
• Excellent organizational, communication, and leadership skills.
• Ability to analyze data, identify trends, and implement effective interventions.
• Proficient with electronic medical records and Microsoft Office applications.

Responsibilities

• Develop, implement, and maintain the facility's Infection Prevention and Control Program.
• Conduct surveillance, tracking, trending, and reporting of infections and communicable diseases.
• Monitor and investigate infection outbreaks and implement corrective actions.
• Ensure compliance with CDC, CMS, OSHA, and state infection control regulations.
• Collaborate with nursing leadership and interdisciplinary teams to reduce healthcare-associated infections.
• Educate and train staff on infection prevention practices, hand hygiene, PPE usage, and isolation precautions.
• Maintain infection control documentation and prepare reports for Quality Assurance and Performance Improvement (QAPI) meetings.
• Conduct audits and provide ongoing monitoring of infection prevention practices throughout the facility.
• Assist with emergency preparedness and outbreak response planning.
• Serve as a resource for residents, families, and staff regarding infection prevention initiatives.
