Job summary

Idaho Department of Health & Welfare seeks a Psychosocial Rehab Specialist (PSR) Service Coordinator for the Infant Toddler Program in Caldwell. The role combines office work and home visits to support families and children, coordinating services and completing required paperwork.

The ideal candidate has PSR certification or a related BA, strong communication, and a detail-oriented approach. The position includes comprehensive benefits and opportunities to work with interdisciplinary teams.

Qualifications

• Minimum: knowledge of human services principles and practices; behavioral management; case management.
• Bachelor-level or completion of PSR certification per USPRA; or related degree with relevant experience.
• Computer literacy for data entry, reporting, and document handling.

Responsibilities

• Conduct intake meetings with families and complete required paperwork.
• Write and review Individual Family Service Plans (IFSPs) for eligible children.
• Conduct IFSP reviews at least every 6 months and coordinate with teams.
• Connect families to community resources and support services.
• Complete data entry and support transitions out of the program.
