Job summary

Vanderheyden Hall Inc. is seeking a Psychosocial Rehabilitation Services provider to deliver PSR to children and youth under 21 who are Medicaid eligible.

Services may be provided in community settings where the youth lives, studies, or receives services to promote independence and community integration. The role includes ensuring compliance with CFTSS and licensure requirements, participating in intake and planning, and supporting families and caregivers in treatment goals and daily living

Qualifications

• High School diploma or equivalent.
• Must be able to lift 25 pounds.
• Must be able to stand and run for moderate periods of time.
• Three years’ experience in children’s mental health, addiction and/or foster care.

Responsibilities

• Comply with all requirements of CFTSS.
• Comply with all requirements of 291 licensure.
• Comply with applicable laws, rules, and regulations, including CFTSS guidelines.
• Participate in the intake and planning process and assessment as requested.
• Provide Personal and Community Competence: Promote independence and community living skills.
• Provide Social and Interpersonal Skills: Build relationships and support networks.
• Provide Daily Living Skills: Develop routines, self-care, and medication management.
• Provide Community Integration: Strengthen social skills and community participation.
• Engage child/youth and family in treatment as described by the plan.
• Deliver services in community locations where the child/youth lives, works or attends school.
