Job summary

Sunflower Diversified Services in Kansas seeks a licensed Physical Therapist to join the Infant Toddler Services program. The PT will work on a transdisciplinary team, provide comprehensive support per the Part C Program Manual, and coach families and staff within their scope of practice.

The role requires Kansas licensure, experience in special education or early intervention, strong communication, and the ability to travel to community sites.

Qualifications

• Must hold Kansas licensure/certification in Physical Therapy.
• Background check clearance.
• Family Service Coordinator certificate obtained after hire.
• Maintain required safety certifications (CPR, First Aid, HIPAA, BBP, Ethics, OSHA).

Responsibilities

• Screen and evaluate developmental skills of children referred to the Infant Toddler program.
• Provide direct services and consultation to families in the program.
• Follow the Mission and Key Principles of Kansas Infant Toddler Services.
• Coach and train team members and families within scope of practice.
• Develop and implement an Individualized Family Service Plan (IFSP) for each child.
• Collaborate with team members and community partners.
• Maintain timely documentation and confidentiality as required by FERPA/HIPAA.
• Attend meetings and trainings as directed by supervisor.
