Job summary

Qualifications

• Massachusetts Physical Therapist licensure required.
• Two years pediatric experience preferred.
• Excellent verbal and written communication skills.
• Proficient with Word, Excel, Adobe, and Office365 (Outlook, OneDrive, Teams).
• Ability to use computer-based technology including a laptop and cell phone.
• Valid driver's license and reliable transportation required.
• Bilingual/Bicultural candidates encouraged to apply.

Responsibilities

• Demonstrate knowledge of infant-toddler development and apply coaching models with families.
• Provide home-based services and participate in screenings, assessments, and visits as part of a trans-disciplinary team.
• Coordinate care across catchment area with community providers to ensure continuity of services.
• Coordinate development, monitoring, and review of Individual Family Service Plans for assigned caseload.
• Collaborate with child care, medical, social service, and specialty providers serving children with autism or hearing/vision loss.
