Job summary

The Columbus Organization, LLC is hiring an Indiana Case Manager to coordinate care for individuals with I/DD across Marion County. The role requires travel within the service area and residence in Indiana, with evening or weekend work possible.

Responsibilities include building relationships with participants and providers, ensuring access to waivers and Medicaid supports, and maintaining compliant documentation and PCISPs.

Qualifications

• Requires Bachelor’s degree in human services-related field or RN with experience in human services.
• Bachelor’s degree plus at least 1 year direct experience with individuals with I/DD satisfies criteria.
• Alternative: 4+ years in human services with at least 1 year I/DD support experience accepted.

Responsibilities

• Serve as Primary Liaison with participants, families, guardians, providers, and state agencies.
• Ensure HIPAA compliance and confidentiality of records.
• Develop person-centered plans (PCISP) aligned with individual goals.
• Conduct annual planning, budget development, and LOCSI determinations.
• Lead IST meetings at least semi-annually with participant and representatives.
• Perform required face-to-face visits, including annual in-home visits.
