Job summary

Community Living Services (CLS) is seeking a Support Coordinator in Michigan to advocate for individuals and families, coordinating IPOS and linking to services and entitlements. The role emphasizes person-centered planning, community connections, and timely documentation.

The position supports driving to served sites with an independent schedule, ensuring access to Medicaid and other benefits while upholding dignity and choice for the people supported.

Qualifications

• Minimum of Bachelor's (or Master’s) degree in Human Services, Social Work, Psychology, Special Education, or a related field.

Responsibilities

• Facilitate the development and implementation of Individual Plans of Service (IPOS) using person-centered planning principles.
• Coordinate, connect, and monitor services and supports identified within the IPOS.
• Assist individuals and families in accessing community resources and benefits, including Medicaid, SSI, Children's Waiver, Autism Benefit, and other entitlement programs.
• Support crisis planning and resolution.
• Coordinate health and safety strategies, including discharge planning.
• Participate in school transition planning and IEP meetings.
• Promote community inclusion, self-advocacy, and relationship building.
• Maintain accurate and timely documentation in compliance with agency, regulatory, and funding requirements.
• Participate in required trainings and professional development
