Job summary

FOX Rehabilitation is expanding in Newburyport, MA, seeking licensed Physical Therapists to deliver one-on-one geriatric care in homes and senior living communities. You’ll assess, plan, and implement evidence-based therapy, collaborating with a multidisciplinary team and using telehealth when appropriate.

Autonomy in scheduling, strong patient relationships, and opportunities for professional growth are emphasized.

Qualifications

• Valid Physical Therapy license in state of practice or eligibility to obtain
• Degree from an accredited Physical Therapy program (DPT)
• Basic computer and EMR proficiency; CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities
• Perform evaluations, develop plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to improve mobility, independence and function
• Document within FOX’s EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
