Job summary

FOX Rehabilitation is growing in Newburyport, MA and seeks licensed Physical Therapists to deliver one-on-one care to older adults in homes or Senior Living Communities. You’ll help patients improve mobility and independence through our Geriatric House Calls model, with autonomy to manage your own schedule and treatment plans.

Qualifications

• Valid PT license in the state of practice or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Current CPR certification
• Proficient with EMR software

Responsibilities

• Provide one-on-one PT to older adults in homes or senior living communities
• Perform evaluations and develop individualized plans of care
• Deliver evidence-based therapy to improve mobility and independence
• Document in FOX EMR following regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff about safety and aging in place
• Provide telehealth when appropriate
