Job summary

FeldCare Connects is seeking an enthusiastic Physical Therapist to deliver quality home health care to patients in La Crescenta and surrounding areas as an independent contractor. You control your own schedule and patient coverage areas with no minimum workload.

As a member of FeldCare Connects, you will determine earnings based on the number of patients treated, with strong per-visit rates and essential administrative support to focus on patient care.

Qualifications

• Active Physical Therapist license in the state.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone.
• Bilingual a plus.

Responsibilities

• Provide home health PT services to patients in their homes.
• Observe, record, and report patient responses to treatment and adjust the plan of care.
• Coordinate with FeldCare Connects team for care coordination and scheduling.
