Job summary

Qualifications

• Graduate of an Accredited Nurse Practitioner program (Masters level preferred).
• Licensed by the Commonwealth of Massachusetts board of registration in Nursing.
• National Board Certification.
• Current active Mass DPH Controlled Substances Registration or ability to obtain post-hire.
• Current active DEA licensure/prescriptive authority or ability to obtain post-hire.
• Maintains BLS certification or appropriate professional certification.

Responsibilities

• Comprehensive, longitudinal clinical and administrative coordination of all care for a specific panel of patients.
• Examination, diagnosis and treatment in a primary care setting, including supervision of care plan and referrals.
• Monitor and manage patients with acute and chronic illnesses; supervise care plans and adjust regimens.
• Partner with patients and families to manage care plans.
• Provide consultation regarding hospitalized patients in the panel.
• Provide direction and support to staff to ensure departmental effectiveness.

Nurse Practitioner program (MS preferred)

Massachusetts nursing license

National Board Certification

Mass DPH Controlled Substances Registration
