Job summary

Millennium Physician Group is seeking a Nurse Practitioner, General / Primary Care to provide comprehensive, evidence‑based primary care across the lifespan in our ambulatory practice. This role works within the APP scope, making independent clinical decisions and collaborating with physicians to deliver high‑quality, patient‑centered care.

You will evaluate and manage acute and chronic conditions, order studies, perform procedures, and educate patients.

Qualifications

• Master's‑level clinical training within the APP scope.
• Ability to work with physicians under the physician collaboration model.
• Strong emphasis on evidence‑based assessment and patient care.

Responsibilities

• Evaluate, diagnose, and manage acute illnesses, chronic diseases, and preventive care.
• Develop individualized treatment plans including meds, immunizations, and screenings.
• Diagnose and manage common primary care conditions such as hypertension, diabetes, obesity.
• Order and interpret labs, imaging, ECGs, and point‑of‑care tests.
• Perform office procedures within scope (wound care, injections, lesion treatment).
• Educate patients and caregivers on diagnoses, medications, and self‑management.
