Job summary

FeldCare Connects is seeking a self-motivated Physical Therapist to deliver premier home health care in the Riverside area. The role involves assessing patients, creating treatment plans, and implementing interventions to improve mobility and reduce pain.

The ideal candidate will be licensed as a Physical Therapist in the state, with the ability to work with adults of all ages and excellent communication skills.

Qualifications

• Must be licensed and registered with the state.
• Ability and enthusiasm to work with all ages.
• Curriculum approved by APTA or CMA/Hospital Council as applicable.

Responsibilities

• Assess patients and evaluate care needs to promote movement, reduce pain, and restore function.
• Determine and implement a treatment plan; deliver interventions and monitor progress.
• Maintain accurate records and communicate effectively with patients and caregivers.
