FeldCare Connects is seeking a motivated Physical Therapist to provide quality home healthcare in Temecula and surrounding areas. As an Independent Contractor, you control your schedule and coverage areas, treating patients of all ages with one-on-one care.

You will document patient responses, determine and adjust the plan of care, and use Cliniconnects for assigning and managing patients. A current PT license, accreditation, reliable transport, and a smartphone are required, with bilingual

Qualifications

• Ability to work with patients of all ages.
• License must be current, active, and unrestricted; conditional or restricted licenses not accepted.
• Completion of an accredited Physical Therapist program.
• Reliable transportation required.
• Smartphone required.
• Bilingual a plus.

Responsabilités

• Deliver quality home healthcare to patients in Temecula and surrounding areas.
• Provide one-on-one treatment and document patient progress.
• Observe, record, and report patient responses to treatment and adjust plan of care accordingly.
• Navigate the Cliniconnects app to accept or decline patients quickly.
• Complete documentation during or immediately after visits.
