Therapy Management Corporation (TMC) is seeking an Occupational Therapist to deliver patient-centered therapy to adults and geriatric populations in a collaborative clinical setting. You will evaluate, plan, and implement OT services, focusing on independence, mobility, safety, and quality of life while partnering with nurses and therapists for optimal outcomes.

Qualifications

• Licensed OT or license eligible in the state of practice.
• Graduate of an accredited OT program.
• Interest or experience in skilled nursing or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and develop personalized care plans focused on functional independence, safety, and quality of life.
• Deliver care through therapeutic activities, ADL training, and adaptive techniques.
• Help patients improve daily tasks such as dressing, bathing, grooming, and meal preparation.
• Partner with interdisciplinary teams, caregivers, and nursing staff to support outcomes and continuity of care.
• Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs.
