Job summary

Therapy Management Corporation (TMC) is seeking an Occupational Therapist to provide patient-centered OT services to adult/geriatric patients in a collaborative clinical setting. You will enhance independence, mobility, safety, and quality of life through personalized therapy plans and measurable outcomes.

Qualifications

• Licensed OT or license eligible in the state of practice.
• Graduate of an accredited Occupational Therapist Program.
• Interest or experience in skilled nursing or geriatric rehabilitation.
• Passion for improving quality of life through patient-centered care.

Responsibilities

• Evaluate patients and develop personalized care plans focused on functional independence and safety.
• Deliver care through therapeutic activities and ADL training.
• Assist patients with dressing, bathing, grooming, and meal preparation.
• Collaborate with interdisciplinary teams, caregivers and nursing staff to support outcomes.
• Track progress, document clinically, and adapt plans as needed.
