Job summary

Therapy Management Corporation (TMC) is seeking an Occupational Therapist to deliver patient-centered OT services to adults and geriatric populations, focused on improving independence, mobility, safety, and quality of life. You’ll work in a collaborative clinical environment with a commitment to meaningful outcomes and autonomy.

In this role, you’ll evaluate patients, develop personalized care plans, and coordinate with interdisciplinary teams to support strong outcomes.

Qualifications

• Licensed OT or license eligibility in the state of practice.
• Graduate of an accredited Occupational Therapist Program.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and develop personalized care plans focused on functional independence, safety, and quality of life.
• Deliver OT services through therapeutic activities and ADL training.
• Support patients in daily tasks such as dressing, bathing, grooming, and meal preparation.
• Collaborate with interdisciplinary teams, caregivers, and nursing staff to optimize outcomes.
• Track progress, document clinically, and adapt plans as needed.
