Job summary

Walmart invites a licensed Independent Optometrist to lease space inside Sam’s Club in Baytown, TX. You will lead eye and vision care, and coordinate general health guidance with autonomy over your practice.

As a lease holder you set your days and hours, control your patient schedule, and keep 100% of exam and professional fees. You may opt into equipment and support services where allowed by state regulations.

Qualifications

• OD from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist MD/DO license may be required.
• Valid state ophthalmology license for practice.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9 form completed.

Responsibilities

• Lead patient care with respect to eye and vision as well as general health.
• Establish days, hours, and patient schedule for your practice.
• Set your fee schedule and manage independent lease responsibilities.
• Provide high quality eye care within a Walmart Sam’s Club environment.
