Qualifications

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• MD or DO in ophthalmology from an accredited school.
• Valid state ophthalmology license.
• General Liability and/or Professional Liability Insurance certificates.

Responsibilities

• Launch your optometry services in Sam's Club and lead patient care in eye and vision health.
• Establish your days of practice and hours of operation at the location.
• Control your patient schedule and fee schedule; retain 100% of exam/professional fees.
