Job summary

Walmart's Sam's Club seeks an Independent Optometrist to launch vision services within the club. You will lead patient care in eye and general health, operating as a lease holder with autonomy over practice days and hours.

You set your fee schedule, manage your patient flow, and retain 100% of exam and professional fees. Equipment and/or Opt-Tech support may be available where state guidelines permit, with a salaried lease model.

Qualifications

• OD from an accredited optometry school and valid state optometry license.
• Ophthalmologist MD/DO from an accredited school with valid license.
• General liability insurance certificate and professional liability insurance.
• Lease agreement requirements and compliance with Walmart guidelines.

Responsibilities

• Establish your days of practice and hours of operation.
• Control your patient schedule and fee schedule.
• Keep 100% of exam and professional fees.
• Manage equipment/instrumentation and access to optional support.

Education

Doctor of Optometry (OD) from an accredited optometry school

Ophthalmologist (MD or DO) from an accredited school

Valid state optometry license

Valid state ophthalmology license

General Liability Certificate

Professional Liability Insurance Certificate
