Walmart’s Sam’s Club is seeking an Independent Optometrist to lease space and deliver eye care. You’ll assume a lead role in patient care for vision and general health within a Sam’s Club setting.

Qualifications

Doctoral degree in Optometry (OD) from accredited optometry school.
Licensed optometrist in good standing in state of practice.
Ophthalmologist (MD or DO) from an accredited school.
Valid state optometry license and valid ophthalmology license.
General Liability Certificate and Professional Liability Insurance Certificate.
W-9 form as required for contractor relationships.

Responsibilities

Establish your days of practice and hours of operation.
Establish your patient schedule and control your workflow.
Set your fee schedule and keep 100% of exam/professional fees.
