As a leaseholder, you can determine practice days, operating hours, and patient scheduling while setting your fees and retaining 100% of exam fees. Optional equipment and admin support may be available where allowed by state laws.

Qualifications

• OD degree from an accredited optometry program.
• MD or DO with ophthalmology license where applicable.
• General Liability Certificate and Professional Liability Insurance required.
• W-9 included as part of the lease process.

Responsibilities

• Establish your days of practice and hours of operation.
• Control your patient schedule within the leased space.
• Set your fee schedule for exams and professional services.
• Keep 100% of exam/professional fees as the lease holder.
• Coordinate equipment/instrumentation and admin support if allowed by state.

Doctor of Optometry (OD) from an accredited optometry school

Ophthalmologist (MD or DO) from an accredited school
