Job summary

Ultra Health Providers seeks a Nurse Practitioner to deliver comprehensive medical care in skilled nursing and senior living settings. You will work with an interdisciplinary team, manage complex cases, and build trusted relationships with residents and families.

You’ll operate with clinical autonomy, providing proactive, patient-centered care and responding to urgent needs to ensure continuity, quality, and dignity in treatment.

Qualifications

• Active, unrestricted NP license in the applicable state(s) of practice.
• Current national Nurse Practitioner certification (ANCC, AANP, or equivalent).
• Clinical training and experience in adult, family, and/or geriatric medicine.
• Eligibility to work in the United States.

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings.
• Independently evaluate, diagnose, and treat acute and chronic medical conditions.
• Conduct initial admissions and follow-up assessments, including physical exams and medication reviews.
• Monitor changes in resident condition and adjust treatment plans accordingly.
• Order and interpret diagnostic tests; prescribe medications and therapies within scope of practice.
• Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams.
• Communicate clearly with residents and families regarding diagnoses and treatment plans.
• Maintain accurate documentation in the EMR in accordance with standards.
• Participate in quality and care coordination efforts to support patient outcomes.
• Adhere to HIPAA, state, and federal regulations while upholding ethical standards.
