Ultra Health Providers delivers onsite medical, mental, and behavioral health services within senior living environments. Our integrated care model improves reaction times, strengthens continuity of care, and reduces avoidable hospital readmissions.

As a Nurse Practitioner with UHP, you will play a vital role in delivering comprehensive medical care to residents in skilled nursing and senior living settings.

Competenze

• Active, unrestricted Nurse Practitioner license in the applicable state(s) of practice.
• Current national Nurse Practitioner certification (ANCC, AANP, or equivalent).
• Clinical training and experience in adult, family, and/or geriatric medicine.
• Eligibility to work in the United States.

Mansioni

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings.
• Independently evaluate, diagnose, and treat acute and chronic medical conditions.
• Conduct initial admissions and follow-up assessments, including physical exams, medication review, and clinically appropriate screenings.
• Monitor changes in resident condition and adjust treatment plans accordingly.
• Order and interpret diagnostic tests; prescribe medications and therapies within scope of practice.
• Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams.
• Communicate clearly and compassionately with residents and families regarding diagnoses, treatment plans, and changes in condition.
• Maintain accurate and timely documentation in the electronic medical record in accordance with regulatory and payer standards.
• Participate in quality and care coordination efforts to support excellent patient outcomes.
• Adhere to HIPAA, state, and federal regulations while upholding ethical standards of clinical practice.
