Job summary

Ultra Health Providers seeks a Nurse Practitioner to deliver comprehensive, onsite medical care to residents in skilled nursing and senior living settings in Madison, OH. You will work independently within an interdisciplinary team to manage complex cases while maintaining patient-centered, high-quality care.

You will collaborate with facility leadership to ensure continuity, timely interventions, and respectful communication with residents and families.

Qualifications

• Active, unrestricted NP license in the state(s) of practice.
• Current national NP certification (ANCC, AANP, or equivalent).
• Clinical training and experience in adult, family, and/or geriatric medicine.
• Eligibility to work in the United States.
• Experience in internal medicine, geriatrics, cardiology, or post-acute care (preferred).
• Active DEA registration or ability to obtain.
• Strong clinical judgment and ability to work independently.
• Excellent communication and interpersonal skills.
• Organized, detail-oriented, and able to manage multiple priorities.
• Prior experience in SNF, LTC, ICU, or ED is a plus.

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and senior living settings.
• Independently evaluate, diagnose, and treat acute and chronic medical conditions.
• Conduct admissions and follow-up assessments, including physical exams and medication reviews.
• Monitor changes in resident condition and adjust treatment plans accordingly.
• Order and interpret diagnostic tests; prescribe medications and therapies within scope.
• Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams.
• Communicate clearly with residents and families about diagnoses and plans.
• Maintain accurate documentation in the EMR per regulatory standards.
• Participate in quality and care coordination to support outcomes.
• Uphold HIPAA and regulatory compliance.
