Job summary

Ultra Health Providers is seeking a Nurse Practitioner to deliver comprehensive medical care in skilled nursing and senior living settings. You will work independently within a collaborative team, manage complex conditions, and build trusted clinical relationships with residents and facility staff.

You will provide proactive, patient‑centered care, respond to urgent needs, and ensure continuity and quality in treatment while adhering to regulatory standards and best practices.

Qualifications

• Active, unrestricted NP license in the applicable state(s) of practice.
• Current national NP certification (ANCC, AANP, or equivalent).
• Clinical training and experience in adult, family, and/or geriatric medicine.
• Eligibility to work in the United States.

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings.
• Independently evaluate, diagnose, and treat acute and chronic medical conditions.
• Conduct initial admissions and follow-up assessments, including physical exams, medication review, and clinically appropriate screenings.
• Monitor changes in resident condition and adjust treatment plans accordingly.
• Order and interpret diagnostic tests; prescribe medications and therapies within scope of practice.
• Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams.
• Communicate clearly and compassionately with residents and families regarding diagnoses, treatment plans, and changes in condition.
• Maintain accurate and timely documentation in the electronic medical record in accordance with regulatory and payer standards.
• Participate in quality and care coordination efforts to support excellent patient outcomes.
• Adhere to HIPAA, state, and federal regulations while upholding ethical standards of clinical practice.
