Job summary

The Nurse Practitioner role offers clinical autonomy, independent decision-making, and collaboration with an interdisciplinary team to deliver comprehensive care that reduces hospitalizations and improves resident outcomes.

Qualifications

• Active, unrestricted NP license in the applicable state(s).
• Current national NP certification (ANCC, AANP, or equivalent).
• Clinical training and experience in adult, family, and/or geriatric medicine.
• Eligibility to work in the United States.

Responsibilities

• Provide primary, urgent, and preventive medical care to residents in skilled nursing and, as applicable, assisted living settings.
• Independently evaluate, diagnose, and treat acute and chronic medical conditions.
• Conduct initial admissions and follow-up assessments, including physical exams and medication reviews.
• Monitor changes in resident condition and adjust treatment plans accordingly.
• Order and interpret diagnostic tests; prescribe medications and therapies within scope of practice.
• Collaborate with facility Medical Directors, attending physicians, nursing leadership, and interdisciplinary teams.
• Communicate clearly with residents and families regarding diagnoses, treatment plans, and changes in condition.
• Maintain accurate and timely documentation in the EMR per regulatory and payer standards.
• Participate in quality and care coordination efforts to support outcomes.
• Adhere to HIPAA, state, and federal regulations while upholding ethical standards.
