Job summary

Community Care Network seeks two Community Outreach Specialists to provide individualized support for residents in independent housing and the surrounding community. You will help clients build living skills, access services, and maintain connections to supports while supporting mental health stability.

Responsibilities include coordinating with clinicians and case managers, providing transportation for appointments, and helping clients progress toward goals in their plans.

Qualifications

• High school Diploma or GED required.
• Bachelor's Degree in Psychology, Counseling, Social Work or similar field preferred.

Responsibilities

• Serve as a role model, advocate, and motivator to individuals in recovery to support long-term stability.
• Assist in planning and implementing clients' care plans by coordinating services with clinicians and case managers.
• Respect individuals' choices and encourage advocacy.
• Provide a professional, individualized support system to supervise and assess current and prospective needs.
• Demonstrate understanding of clients' physical health, mental health, and psychosocial factors to address needs.
