• Assist in developing individualized, person-centered service plans and implement them
• Provide support with daily living activities, skill development, and personal goal achievement

• Implement activities that promote community inclusion, relationship building, and personal growth
• Maintain a safe, clean, and supportive environment for individuals served
• Review, complete, and maintain all required documentation accurately and timely
