• Assist in developing individualized, person-centered service and support plans and help implement them.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with transportation and access to community resources as needed.
• Implement activities that promote community inclusion and meaningful social engagement.
• Maintain a safe, clean, and supportive environment and complete required documentation accurately.
