Job summary

Open Sky Community Services in Massachusetts is seeking a Residential Counselor to support individuals in residential and community-based settings. You will provide direct care, guidance, and skill-building to promote independence and meaningful daily living.

Responsibilities include developing person-centered plans, assisting with daily living, transportation, documenting progress, and advocating for community inclusion. A high school diploma and valid driver's license are required.

Qualifications

• High School Diploma, GED or equivalent is required.
• Valid Driver’s License and acceptable driving record required.
• Reliability, willingness to learn, and being open to new opportunities.

Responsibilities

• Assist in developing individualized, person-centered service and support plans and play an active role in their implementation.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with accessing transportation and community resources as needed.
• Implement activities that promote community inclusion, relationship building, personal growth, and the development of valued social roles.
• Maintain a safe, clean, and supportive environment that promotes the health, safety, and well‑being of individuals served.
• Review, complete, and maintain all required documentation in an accurate and timely manner.
