Qualifications

Responsibilities

• Assist in developing individualized, person-centered service and support plans and contribute to their implementation.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with accessing transportation and community resources as needed.
• Implement activities that promote community inclusion, relationship building, personal growth, and valued social roles.
• Maintain a safe, clean, and supportive environment for individuals served.
• Review, complete, and maintain all required documentation accurately and timely.
