Responsibilities

• Coordinate and provide individualized ISL services promoting independence and growth.
• Collaborate with Program Coordinators, nursing staff, guardians, and support teams to ensure high-quality care.
• Monitor health needs by coordinating medical appointments, documenting outcomes, and following up on recommendations.
• Train and support staff regarding medical needs, medications, diets, adaptive equipment, and personal preferences.
• Maintain daily residential documentation, goal tracking, and health monitoring.
