Job summary

Brightli is seeking a Program Coordinator in Kansas City, MO to lead and coordinate individualized support services in home and community-based settings. You will mentor staff, ensure quality care, and promote independence and community involvement.

Responsibilities include ISP development, staff supervision, and collaboration with nurses, families, and other stakeholders to achieve positive outcomes. A Bachelor's degree is required, with background checks and driving requirements noted.

Qualifications

• Bachelor's degree from an accredited college or university required.
• Relevant experience in Developmental Disabilities may be substituted for education on a year-for-year basis.
• Background screenings including criminal history, driving record, abuse/neglect, and fingerprint checks required.

Responsibilities

• Coordinate the development, implementation, and monitoring of Individual Support Plans (ISPs) to ensure meaningful and measurable outcomes.
• Support individuals in achieving personal goals while promoting independence, community participation, and natural support systems.
• Lead, mentor, supervise, and evaluate Community Service Managers, Primary Support Staff, and Direct Support Staff.
• Collaborate with support teams, nurses, families, and other stakeholders to ensure individuals receive quality care and services.
• Monitor the health, safety, and well-being of individuals served through regular site visits and ongoing oversight.
• Coordinate medical services and wellness initiatives, including support for healthy lifestyles, medication oversight, and treatment compliance.
• Ensure team members receive required training and remain knowledgeable about individual support plans and service expectations.
• Review records, therapy documentation, and service documentation to ensure accuracy, completeness, and compliance.
• Conduct audits and quality reviews, including documentation reviews, progress monitoring, and record assessments.
• Maintain client records, staffing information, budgets, and related service data within organizational systems.
• Monitor budget performance, identify variances, and assist with annual budgeting processes.
• Ensure residential procedures, money counts, and controlled medication counts are completed appropriately when applicable.
• Maintain compliance with organizational policies, HIPAA requirements, and regulatory standards.
