Job summary

Vinfen is seeking a dedicated staff member to support persons served in a Vinfen setting. You will help develop and document individualized treatment plans, perform daily tasks, and assist with community integration.

You will assess health status, maintain safety, and manage medication and logs per MAP requirements, with driving duties possible. A high school diploma and basic certifications are required, with physical ability to lift 25 pounds.

Qualifications

• One to three months experience in a human services setting.
• A high school diploma or equivalent is required.
• Must be able to lift at least 25 pounds.
• May be called to drive for Vinfen using a Vinfen van or personal vehicle.

Responsibilities

• Assist in developing, implementing, and documenting individual treatment support plans.
• Perform a variety of daily tasks with persons served, including interventions, monitoring, and transportation as needed.
• Provide role model and support to persons served to help them integrate into their communities.
• Assess physical and emotional status of persons served and report changes to supervisor.
• Maintain program site safety and assist with daily living tasks and cleaning.
• Comply with corporate policies and safety practices; document and dispose of medication per MAP requirements.
• Complete paperwork and logs such as timesheets and forms.
• Perform crisis intervention as needed.
• Provide updated contact information to RelieF Pool Coordinator or HR.
• Perform other duties as required.
